PRE-REGISTRATION

GenLife Institute encourages individuals who choose whole-body donation to pre-register; however, pre-registration is not required to become a donor.

To become a registered donor, you must complete a donation and cremation authorization. These forms can be downloaded here:

If you would like an informational packet to be mailed to you, please complete the electronic information request or contact our office at (877) 738-6111. Once your request is received, a packet will be mailed within 48 hours.

AT THE TIME OF PASSING

If donation is chosen at the time of passing, please contact GenLife Institute immediately at
(877) 738-6111. Once GenLife Institute is contacted, our dedicated staff will guide the family through the donation process which will include a brief medical-social history and required consent forms.
